The term “acne” refers to a process which can result in as few as one small blackhead to severe cystic acne. The initial acne lesion involves a build-up of keratinized cells in the follicle lining, which is thought to occur due to an overproduction of sebum. The sebum and keratinized cells cause a blockage or obstruction, which expands the follicle. This process is one of the key factors responsible for acne. With the reduction keratosis, follicular occlusion could be prevented.

The Acne Process
The surface of the skin is covered with pilocebaceous follicles, which are long, hollow tubes not much wider than the diameter of hair. The walls of these follicles are carpeted with the same flat, dead cells that compose the stratum corneum. As a result, thousands of microscopic cells are being shed. 
Normally, oil carries these cells out of the follicle to the surface where they are eventually eliminated. However, in individuals who are genetically prone to acne, these cells cannot reach the surface because a “glue like” substance causes them to stick together. 
In the beginning of the acne process, the glue like substance initiates a microscopic clump of dead skin cells to stick together called microcomedone. During the initial acne stage, the process occurs exclusively within the follicle. The surface of the skin may appear clear and unaffected. 
As acne progresses, the microcomedones trap oil within the follicle. The trapped oil is “eaten by the P. Acne bacteria which then excrete fatty acid metabolic by-products. These fatty acid by-products are inflammatory and corrosive. Over time ranging from several weeks to many months, the follicle may become completely clogged. At that point, various lesions may begin to surface but not necessarily in that order. Back



Why Are Some Individuals More Prone To Acne?
Scientists agree that the 4 major causes of acne are: 1) keratosis, 2) oil Secretion, 3) bacteria and 4) androgen hormones. It is not known exactly why acne occurs, but electron microscopes have shown that individuals prone to acne have an abnormally high level of cellular keratinization ion inside the lining of the follicular canal. In other words, the follicles fill up with dead cells and become easily clogged. 
In addition, the corneocyte cohesion of acne prone individuals is abnormal, causing the 
corneocytes to be more densely pact and more firmly bound together than the corneocytes of individuals who are not prone to acne. After in-depth studies, scientists have established that the material extracted from these follicles contain colonies of Propionibacterium acnes (known as P. Open Comedones or “Blackheads

Open Comedones commonly referred to as Blackheads, result when the opening of the follicle expands to accommodate the accumulation of dead cells, oil, etc. Although the follicle is hot completely blocked, blackheads or sometimes merely enlarged or clogged follicles commonly referred to as pore openings, can be seen. Anything clogging the follicle can stretch the opening, making it appear enlarged. To prevent the follicular build from occurring, the follicle must be cleansed on a continuous basis.

Closed Comedones

The good news about an open come done is that as long as the follicle has an exit or opening, the condition will usually not progress beyond the open comedone stage. Closed Comedones, on the other hand, often develop when the opening of the follicle is completely blocked or closed by either a hard plug or physical pressure, and are often visible as small flesh colored bumps under the skin. Even though they may not appear inflamed or particularly active, they signal the probability that a more serious stage of acne is taking place beneath the surface. Back
